Description

Omnimed preprinted Poly chart dividers are made of strong, flexible heavy gauge polyethylene plastic. Our fade resistant dividers are printed in black on one side using an 8 point foil stamping process that produces a clean and easily readable Type. These sets are top open 5 hole punched and ship in collated packages. Dividers measure 8 1⁄2 inches wide by 11 inches long and are compatible with other charting systems. Tab copy option 3: advance directives (white), care plan/MDS (turquoise), admission records (Yellow), history & physical (Green), physicians’ orders (Red), progress notes consults (gold), nurses’ notes (Orange), medication & treatment (Blue), lab & x-ray (gray), rehab & therapy (peach), social services (Lime), dietary (lt. Blue), activities (Mauve) and Misc. Records (Lavender).
